Misellaneous costs

Payment is due at time of service. We accept payment by credit card, debit card, flexible spending account, cash, money order, or check.  

  • $15 for FMLA/Disability paperwork; and any letters written on your behalf to your school, lawyer, employer.
  • $20 for medication refill without the evaluation
  • $20 for copying records, unless requested by another healthcare provider.
  • $15 for any medication requiring prior authorization.
  • There may be a $40 fee if you miss your scheduled appointment without cancelling at least 24 hours in advance.
  • If your account is referred to a collection agency for non-payment, you will be discharged from the practice and a 30% service fee will be added to your account balance.
  • $25 returned-check fee

Appointment Visit Costs

I offer more flexible times and a discounted rate with secure online telemedicine visits.  Private live visits replace traditional office visits for non-emergency evaluations. Initial visit $125; Established telemedicine visit $70

1st office visit without insurance $125; established return office visit $85

If you are using your insurance benefits for traditional office visits, your insurance sets the price. If you are not eligible at the time services are rendered, you are responsible for full payment. We file your insurance for office visits as a courtesy.  Your out of pocket cost for the office visits depends on the rules of your insurance plan. Using insurance, your first visit can cost a range from $0 up to a maximum of $110; a return office visit can cost a range from $0 up to a maximum of $100, depending on your plan. You can call your insurance customer service number to inquire.
